How to File a Claim



To Report a Claim by Telephone

We welcome initial reports of claims by telephone and many of our customers prefer this method. It is fast and allows for immediate human interaction. Claims representatives are available to assist you with any type of claim during regular business hours and through our answering service at any time for emergencies. When you call to report a claim, our claims representative will ask questions similar to those that are found on the claims forms.

To report a claim or to check the status of a claim by phone, call (800) 554-2642, then select Option "2" when prompted.

Non-emergency claims may be reported during regular business hours, which are 7:00 a.m. until 5:45 p.m. Central time, Monday through Friday.

Emergency claims may be reported at any time. Church Mutual claim staff remain on-call round-the-clock and may be reached through our emergency answering service.

Before you call, it is helpful if you have your account and policy numbers, a description of the claim or accident, phone numbers where you may be reached, and any other facts regarding the claim that may help our claims representative process your claim promptly.

A Church Mutual claims representative may process your claim directly with you, or they may assign an adjuster in your area to assist in handling your claim. The adjuster reports to the claims representative, who will oversee the claims process for you. So, either way, you will receive prompt, fair and courteous treatment. If you have questions during the settlement process, you may call Church Mutual at (800) 554-2642 (then press Option "2" when prompted).

Claims reporting forms are included with every Church Mutual policy. You can fax these forms--or forms you have downloaded from this Web site--to Church Mutual's Claims Department at (715) 539-4651.

To Report a Claim Using E-mail

You can use e-mail to notify us of a claim and to correspond during the settlement process. Initial e-mail claims reports should be sent to claims@churchmutual.com. Upon receipt, we will direct your e-mail to the appropriate claims representative.

If you notify us via e-mail, please provide a description of the damage or injury, including how and when it occurred. For injuries, provide the name, address and phone number of the injured person(s). Please also provide your name, policy and account numbers and the name and address of the Named Insured on your policy. So that we can respond to you in a timely fashion, include your phone number and the best time to reach you.
