
- Go to ScreenNow, a ChoicePoint® service, by clicking the "Request a
Background Screening" link at the bottom of the page. This will open the ScreenNow Pre-Employment Screening Subscriber Application and Service Agreement form.
Important Benefit: By going through the Church Mutual Web site to apply for a ScreenNow account, the normal $175 account set-up fee will be waived, and you will receive deeply discounted prices on screening services.
- Complete all of the information requested on the online application form and submit it by clicking the "Process my Application" button toward the bottom of the page.
Note: On the application, be sure to include a telephone number where you can be reached for verification during the day (8 a.m. to 5 p.m. EST).
- After submitting the initial application form, a ChoicePoint sign-up form will appear on your screen. Please print, complete, and fax this document to ChoicePoint at (678) 893-9600. If you have 501(c)3 documentation, please also fax that.
Key: Including your 501(c)3 documentation will make the approval process go more smoothly and quickly.
- Shortly after you submit your application and sign-up form, a ChoicePoint representative will call you to verify your request.
- If you provided 501(c)3 documentation, you have completed the process and will receive a welcome e-mail from ScreenNow. However, if you did not provide that documentation, you likely will receive a personal visit to your location so that ChoicePoint can verify you are a legitimate organization that will responsibly use their data.
