Important information about opening a ScreenNow® account

  1. Go to ScreenNow, a ChoicePoint® service, by clicking the "Request a Background Screening" link at the bottom of the page. This will open the ScreenNow Pre-Employment Screening Subscriber Application and Service Agreement form.

    Important Benefit: By going through the Church Mutual Web site to apply for a ScreenNow account, the normal $175 account set-up fee will be waived, and you will receive deeply discounted prices on screening services.

  2. Complete all of the information requested on the online application form and submit it by clicking the "Process my Application" button toward the bottom of the page.

    Note: On the application, be sure to include a telephone number where you can be reached for verification during the day (8 a.m. to 5 p.m. EST).

  3. After submitting the initial application form, a ChoicePoint sign-up form will appear on your screen. Please print, complete, and fax this document to ChoicePoint at (678) 893-9600. If you have 501(c)3 documentation, please also fax that.

    Key: Including your 501(c)3 documentation will make the approval process go more smoothly and quickly.

  4. Shortly after you submit your application and sign-up form, a ChoicePoint representative will call you to verify your request.

  5. If you provided 501(c)3 documentation, you have completed the process and will receive a welcome e-mail from ScreenNow. However, if you did not provide that documentation, you likely will receive a personal visit to your location so that ChoicePoint can verify you are a legitimate organization that will responsibly use their data.
